We are thrilled you are attending our event and want you to have the best experience possible! Most events have been canceled due to the pandemic. At Keaton’s Child Cancer Alliance, we felt it was critical to continue the important work that we do by moving forward with our annual golf tournament. We want to assure you that many discussions have taken place to ensure that our event meets the current regulations and your safety is our top priority.

Due to the contagious nature of the Coronavirus/COVID-19, the CDC and many other public health authorities are still recommend practicing social distancing (or wearing a face mask when social distancing is not possible) and utilizing hand washing/sanitizing. Keaton’s Child Cancer Alliance has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19, but there is still an inherent risk.

You are voluntarily opting to play in the “Chipping Away at Childhood Cancer” golf tournament, hosted by Keaton’s Child Cancer Alliance, and acknowledge that this increases your risk of exposure to the Coronavirus/COVID-19, particularly if you are over the age of 60 or have a pre-existing condition. By complying with all set procedures, it will reduce the spread while attending the event.

You attest that: * You are not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell. * You have not traveled internationally within the last 14 days. * You have not traveled to a highly impacted area within the United States of America in the last 14 days. * You do not believe you have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. * You have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities. * You are following all CDC recommended guidelines as much as possible and are limiting your exposure to the Coronavirus/COVID-19.
